The Second Hand Smoke Complaint Form (1page) is used to file complaints about all violations of RSA 155:64-77, New Hampshire's Indoor Smoking Act. Completed forms should be submitted to the Department of Health and Human Services, Tobacco Prevention & Control Program for investigation.
