The Report on Infants and Toddlers Exiting Part C Programs (4 pages) is a 12 month report that provides information on the following:

•  the number of infants and toddlers served by area agencies in twelve separate regions of the state through the Family Centered Early Supports and Services Program;
•  the number of infant and toddlers exiting the Family Centered Early Supports and Services program for the following reasons:
  • Completion of IFSP prior to reaching 36 months of age
  •  Eligible for Special Education (Part B)
  • Not Eligible for Special Education (Part B) at 36 months
  • Eligibility for Special Education (Part B) not determined by 36 months
  • Deceased
  • Moved out of state
  • Withdrawal by Parent
  • Attempts to contact unsuccessful
    

This report will be updated and available annually beginning State Fiscal Year 2009.
