The Report on Infants and Toddlers Exiting Part C Programs (4 pages) is a 12 month report that provides information on the following:
- the number of infants and toddlers served by area agencies in twelve separate regions of the state through the Family Centered Early Supports and Services Program;
- the number of infant and toddlers exiting the Family Centered Early Supports and Services program for the following reasons:
- Completion of IFSP prior to reaching 36 months of age
- Eligible for Special Education (Part B)
- Not Eligible for Special Education (Part B) at 36 months
- Eligibility for Special Education (Part B) not determined by 36 months
- Deceased
- Moved out of state
- Withdrawal by Parent
- Attempts to contact unsuccessful
This report will be updated and available annually beginning State Fiscal Year 2009. |
