Assisted Living and Residential Care Services:
An individual qualifies for Assisted Living or Residential Care Services through the Home and Community Based Care program for the Elderly and Chronically Ill (HCBC-ECI) Medicaid waiver program. This is a four-step process.
- An individual must apply for the HCBC-ECI program. This process starts at a DHHS district office. An individual must have medical needs that require nursing facility level of care. A Bureau of Elderly and Adult Services (BEAS) health care professional determines if an individual meets the medical criteria for nursing facility level of care and whether HCBC-ECI is an appropriate and cost effective option.
- When an individual is found to meet the nursing facility level of care criteria, he or she meets with a nurse and a case manager to determine eligibility for HCBC-ECI services.
- The nurse, case manager and individual jointly develop a service plan. It is critical that services be locally available to support the service plan. Case managers meet with the individual and consult with BEAS nurses on an on-going basis.
Congregate Housing Services:
An individual qualifies for Congregate Housing Services through the Home and Community Based Care program for the Elderly and Chronically Ill (HCBC-ECI) Medicaid waiver program and they must meet the following criteria:
- An individual must be 62 years of age or older
- Live in a municipal or public housing facility that participates in the Congregate Housing Support Services program
- Be unable to perform at least three activities of daily living such as bathing, dressing, eating or toilet use.
