Respite and Support Services to people of any age who provide care for an older family member and also to older persons who provide care for a grandchild.
- The individual receiving care must be age 60 and older.
- The individual receiving care must be age 18 or younger and the primary caregiver must be age 60 or older (grandparents, other relatives, legal guardians, or others).
- The individual receiving care must require assistance with at least two activities of daily living, e.g., bathing, dressing, eating and toilet use.
- There are no income requirements.
Alzheimer's Disease and Related Disorders Programs for Individuals diagnosed with ADRD and family members who provide day-to-day care to individuals with ADRD.
- A physician certifies that an individual has ADRD.
- A primary caregiver lives with an individual who has ADRD and provides care 24 hours per day without pay.
- A primary caregiver completes the specified application.
