Eligibility

Home & Community Services

 

Eligibility is based on many factors in addition to income related criteria, including age or disability, residency and in some cases meeting skilled nursing care requirements. 

 

For more detailed eligibility information call any DHHS district office or the ServiceLink Resource Center.  Certain income requirements or qualification of the following programs may also apply: 

 

MEDICAID This program provides access to medical services.   Eligibility is based on financial, resources and medical condition.

 

 

HOME COMMUNITY BASED CARE for the ELDERLY & CHRONICALLY ILL (HCBC-ECI)-  A Medicaid program that provides services designed to support independent living for aging adults and adults with disabilities or chronic illness. To be eligible for the Home and Community Based Care for the Elderly and Chronically Ill (HCBC-ECI) program an individual must meet financial as well as medical eligibility criteria.

 

 

  • When determined eligible for Medicaid an individual can then apply for the HCBC-ECI program.  Medical information will be requested as part of the eligibility process at the DHHS District Office.  To receive HCBC-ECI services an individual must meet criteria for nursing home level of care in addition to the financial eligibility of Medicaid.

 

  • An individual meets with a nurse and case manager to review medical information and other information as needed to determine if an individual will receive HCBC-ECI program services.

 

  • The nurse, case manager and individual develop a service plan when all eligibility requirements are met.  Case managers meet with an individual and consult with nurses from the Bureau of Elderly and Adult Services on an on-going basis.

 

  • Needed services must be available locally to support the service plan.  

 

SOCIAL SERVICES BLOCK GRANT (SSBG)- Eligibility is based on a monthly income limit of $1116.00 with no limits on resources or expenses.  The individual must be an adult age 60 or older or an adult age 18 or older with a disability or chronic illness.  A Bureau of Elderly and Adult Services (BEAS) social worker assists with determining service needs and meets with the individual on an on-going basis to determine well-being and required level of care.
