How do family caregivers find supports for themselves and the people they care for?  The answer is ServiceLink, a statewide network of community-based offices that are one phone call away.  Their friendly staff will listen to your needs, respect your privacy, and help you find answers.

ServiceLink is a partnership between the New Hampshire Department of Health and Human Services and your community's human services providers.

 

What services will the ServiceLink Resource Center provide:

 

  • Information, referrals and assistance via phone, email, personal meetings;
  • Needs assessments;
  • Eligibility pre-screenings and counseling for programs and services;
  • Face-to-face long-term supports counseling to assist people considering nursing home admission to become more aware of their alternative *community-based options and services;
  • Streamlined access to clinical level of care determinations required for Medicaid in-home care or nursing home eligibility;
  • Assistance navigating the financial eligibility processes for Medicaid Long Term Supports (LTS) benefits;
  • Comprehensive caregiver support to those caring for family members in the community; and
  • Other planning and preventative long term supports education programs.

 

 

You can access your local ServiceLink Resource Center Team by calling the national toll fee number, email, walk-in, appointment, or letter.  There is a community-based office in every County of the State of New Hampshire.

  • Medicare Learning Centers to help you understand your benefits.  These centers have a computer with Internet access and a lending library for you to use; and
  • Family Caregiver lending library supplied with useful tools and resources that can be mailed to your or available to you to access at the ServiceLink Resource Center sites.
