The Licensing Application for Supported Residential Care and Residential Care Facilities  (8 pages) is required to be completed and submitted to the Bureau of Health Facilities Administration.  This application should be used for facilities licensed or to be licensed as Assisted Living Residence-Supported Residential Health Care Facilities and Assisted Living-Residential Care Facilities only. Includes new fee schedule effective 7/1/2009.
