The Resident Assessment Tool (RAT) (5 pages) is used to assess the needs of residents in Assisted Living Facilities. The RAT is completed prior to admission, every 6 months or when there is a significant change in the needs of the resident and is maintained in each resident's record at the Assisted Living Facility.
