The Division of Public Health Services (DPHS) has limited funds to pay for specific anti-TB patient care for eligible NH residents. Services covered include outpatient procedures related to the diagnostic work-up and treatment of tuberculosis disease or infection such as office visits, chest x-rays, lab tests and TB medications. Only services directly related to the diagnosis of tuberculosis will be covered for the duration of the patient's therapy. All reimbursements will be at the current Medicaid rate. Inpatient hospital and emergency department services will not be reimbursed by this program.
An individual must reside in NH and meet medical and financial requirements.
MEDICAL -- An individual must:
- be infected with TB or have undergone diagnostic procedures because of suspected TB, and
- be under the care of a physician.
FINANCIAL -- An individual must:
- be ineligible for Medicaid;
- not have private health insurance that would pay for these services; and
- have annual gross household income less than 200% of the Federal Poverty Guidelines, except where household medical debt exceeds guidelines.
If your income is greater than allowed under the federal poverty guidelines, the difference shall be multiplied by 80% to determine the dollar amount of medical bills an individual must pay out-of-pocket in the prior 12 months before he or she can be eligible for assistance from this program.
Public Health Nurse Coordinators who work with DPHS provide patients with an Application for Assistance form. All information provided is strictly confidential. Forms must be returned to the TB Program for eligibility determination. Patients approved for financial assistance are provided with a voucher for services that is valid for the duration of therapy, up to a 13 month maximum. When approved, an individual is asked to provide invoices from his or her health care provider for TB services they receive.
