Health Care Providers can use the following forms for reportable diseases. The forms may be faxed or mailed. Arboviral Illness Human Case Report Form (1 page) Disease Reporting Form (1 page) HIV/AIDS Adult Case Reporting Form (2 pages) HIV/AIDS Pediatric Case Reporting Form (4 pages) Lyme Disease Case Reporting Form (1 page)
