The New Hampshire Guidelines for Diabetes Care (66 pages) is a series of information sheets developed to assist providers when caring for patients with diabetes. The clinical information contained within the guidelines is primarily based upon the recommendations of the American Diabetes Association, but is presented in a format designed to facilitate easy application in the medical office setting. Materials are available here and from the Diabetes Education Program without copyright restrictions. Charts and forms may be freely copied and shared. Useful references such as a listing of diabetes support groups and a directory of Certified Diabetes Educators are also provided.
