Do you have a resident, or two, that could be better served in an alternate setting?  Do you know a resident that has responded so well to living in a nursing facility that returning home should be an option but it has not happened due to lack of resources.? Do you have a resident that this applies to but does not have a home to go back to?  If any of this makes you think of any of your residents, you need to attend this program.  The program will give you information on how this project started, how it will potentially affect your residents and staff and what resources are available to help you help them out.

 

New Hampshire Community Passport (NHCP) is a nursing home transition initiative developed through the State's Money Follows the Person Grant.  It is intentionally designed to reflect the values inherent in the State's long term care vision statement by implementing a person-centered approach to service planning and delivery and providing greater opportunities for consumers to direct their own care to the extent that they choose.
