NH Community Passport-A Money Follows the Person Initiative

The New Hampshire Community Passport Program (NHCPP) is a nursing home transition initiative made possible through the Centers for Medicare and Medicaid Services, Money Follows The Person (MFP) Rebalancing Demonstration Grant.  This program is designed to reflect the values inherent in the State's long term care vision statement by implementing a person-centered approach to service planning and delivery and providing greater opportunities for consumers to direct their own care to the extent that they choose.

Persons who are eligible for home and community based services and reside in qualified institutions will be eligible to participate in the Community Passport program. This includes person's eligible for the developmental disabilities waiver, the acquired brain disorder waiver, the elderly and chronically ill waiver, state funded mental health services and children eligible for the In Home Support Waiver.

This special initiative will focus on:

  • Individuals who have been residing in qualified institutions for a minimum of 6 months.
  • Individuals who have been receiving Medicaid for at least 1 month.
  • Individuals who voluntarily want to transition back into the community and are willing to play an active role in their plan of care.

The goals of the program are:

  • To increase the use of home and community-based long term care services.
  • To eliminate barriers that prevent or restrict the flexible use of Medicaid funds to enable Medicaid-eligible individuals to receive support for necessary long-term services in the settings of their choice.
  • To increase the ability of the State Medicaid Program to assure continued service of home and community-based long term services to eligible individuals who choose to transition from an institution to a community setting.
  • To ensure that procedures are in place to provide quality assurance for individuals receiving Medicaid home and community-based long term care services and to provide for on-going improvement in such services.
