NH Community Passport – Provider Information
 

The New Hampshire Community Passport (NHCP) is a nursing home transition initiative made possible through the Centers for Medicare and Medicaid Services.  The provider information on this page includes data and resources that can assist providers and stakeholders in learning more about this initiative.

 

 

Key Milestones –

10.01.2007 - The Centers for Medicare and Medicaid Services approves the Operational Protocol for NH Community Passport, a Money Follows the Person Initiative. The NH Operational Protocol provides a comprehensive overview of the initiatives goals, objectives and project design. 

 

 

03.05.2008  - NHCP announce it's new project partners.  NH Independent Case Management Association, the NH Health Care Association and Granite State Independent Living have joined together under a contract with the Bureau of Elderly and Adult Services.  This collaboration will focus on education, training, marketing and provide relocation services under this important initiative.
