The Employment Verification Form 756 (1 page) is used by the Division of Family Assistance to collect needed information and provides accurate wage information from the employer's payroll files, provides verification of payroll deductions, and collects information about terminated employment. 

Employment Verification is a part of the application and redetermination process for Food Stamp benefits and all categories of financial and medical assistance.  Employment information must be verified before a determination of eligibility can be made.
