To be eligible for admission to GHE an applicant:

• Must be 60 years of age or older, or younger if the GHE superintendent determines that the applicant:
• Must be referred to GHE by an Area Agency or Community Mental Health Center;
• Has an Individual Service Plan (ISP) that identifies GHE as the least restrictive alternative capable to provide the level of care the person requires; and
• Meets all of following criteria:

1. Has a mental illness or developmental disability;
2. Does not require physician services or psychiatric care on a daily basis;
3. If a medical assistance recipient must qualify for nursing facility care; and
4. Voluntarily requests admission or a guardian requests admission with probate court approved admission request is received from a guardian
