The Confidential Data Request Form (37 pages) must be filled out completely. Any areas left blank without explanation will delay review of this request. The approval process generally takes approximately four to eight weeks from the time your completed form is received. The committee meets monthly to review requests. You will be notified of the status of your request after the monthly meeting.
