The Non-Confidential Data Analysis Request Form (3 pages) is required for requests for non-confidential data regarding vital records, hospital data and cancer registry. Request must include a description of analysis requested, geographic area covered, data elements including years, outline of format for report and a description of why the data is requested including how it will benefit NH residents.
