Initial Registration Requirements:

• Submit an ophthalmic dispensing registration application and pay the required registration fee.

 Renewal License Requirements:

• Submit an ophthalmic dispensing license renewal application and pay the required registration fee; and
• Provide documentation that 8 hours of continuing education was completed in accordance with NH Administrative Rules.  Five of the 8 hours must be through live, didactic courses.
