Adult Family Care is a combination of supervision, personal care, homemaking and other services that are provided to a person in the licensed or certified home of another individual. Adult Family Care does not include care provided to seniors by adult children or their spouses.
Adult Family Care is available as part of the Home and Community Based Care for the Elderly and Chronically Ill (HCBC-ECI) program. The HCBC-ECI Program is part of the NH Medicaid Program. It serves elderly and/or frail adults who qualify for nursing facility care because of their medical needs. Individuals also have to meet the financial eligibility requirements for the Medicaid Program. The HCBC-ECI Program enables individuals to live in their own homes, or in an assisted living or residential care facility. Currently, the Adult Family Care Program is available only in Hillsborough County. The Adult Family Care Program is a new type of residential option for eligible individuals needing care and support. Its development is ongoing and is part of the Department's goal to expand community based care options.
Community Partners
The Area Agency of Greater Nashua and Moore Center Services in Manchester provide oversight of Adult Family Care providers. They recruit and screen potential providers and train providers. Adult Family caregivers must have their own homes, through ownership or rental, and must be licensed or certified by the Department. The licensing or certification process includes a criminal background check and a life safety inspection of the home.
