HomeCare Works is a proposed GraniteCare initiative that provides a system of in-home support services for NH's seniors and disabled adults who are able to live independently but require assistance with daily activities. The program works hand-in-hand with the New Hampshire Employment Program by providing career opportunities in the home health care field to individuals receiving state and federal assistance, in exchange for a commitment to provide care for seniors in New Hampshire.
Home Health Care Careers & Training:
The HomeCare Works program is federally funded and a network of community partners work closely with DHHS to develop, deliver and maintain quality in-home care training at the highest level of health care standards. Training encompasses both the technical and soft skills, such as, job etiquette, financial management and how to balance work and family.
Participants are provided the with the opportunity to advance their training from in-home care support to pursuing a career as an LNA, LPN, or RN. Training and additional career advancement is provided from within the existing infrastructure of trainers recognized by the NH Board of Nursing.
Employment & Job Placement:
A strong working relationship with local direct care staffing agencies is a part of their role on the local health care consortium. In addition to participating in the training design, these employers also provide on-the-job-training and referrals for placement in full and/or part-time employment within their communities.
Working with Communities:
This program will be developed and delivered as a partnership between health care consortium groups to be organized within each delivery area, and the NH Department of Health & Human Services (DHHS). Consortium groups will serve in an advisory capacity as well as an employment link to the health care community.
DHHS Programs & Partners:
The program will be administered and coordinated through the DHHS NH Employment Program (NHEP) state and local office staff. Within DHHS, the Bureau of Elderly and Adult Services will partner with the Division of Family Assistance on the further development and implementation of this program.
