The GraniteCare Senior Wellness Initiative supports NH Senior Centers to develop, enhance and sustain wellness programs that assist older adults in understanding and choosing healthy lifestyles.  With a focus on education, prevention and wellness, this program helps to reduce the burden of illness, enhance quality of life and increase longevity through daily choices and activities.  

 For SFY 2007, Senior Centers proposed to implement one or more wellness initiatives in the following subject areas:

• Chronic Disease Self-Care (diabetes, heart disease, hypertension, arthritis, etc.)
• Physical Activity
• Falls Prevention
• Nutrition and Diet
• Depression and/or Substance Abuse

This initiative was a competitive grant opportunity available to senior centers in New Hampshire and the New Hampshire Association of Senior Centers. Through the competitive grant process, awards totaling $60,000 were made in SFY 2007 to 27 senior centers statewide, including the NH Association of Senior Centers, to support these activities.   Established by House Bill 1, 2005, for SFY 2006 and continued with support by the Legislative Fiscal Committee for SFY 2007, funds were appropriated to support senior wellness programming with a primary focus on prevention.  The following centers were funded in the 2007 Senior Wellness initiative:
