House Bill 1, House Bill 2 - State Operating Budget for State Fiscal Years 2006-2007. Appropriates funding and provides approval for the Department of Health & Human Services to carry the GraniteCare initiatives outlined in the GraniteCare Proposal.
House Bill 691 - The purpose of this act is to carry out the proposals contained in the GraniteCare plan for the modernization of the Medicaid program as it relates to the provision of long-term care with a new emphasis on maintaining our elderly in home and community-based care through the empowerment of consumer choice.
Fiscal Committee Approvals
GraniteCare Highlights of the 2008-2009 Budget Request
- Includes $6 million in Deficit Reduction Act (DRA) Systems Transformation Grants that the Department has applied for which will be directed at information technology necessary to support the Medicaid program.
- Includes savings of $2.9 million from helping individuals to move out of institutions and into community based care referred to as federal Money Follows the Person Program.
- Includes savings of $7.3 million related to the GraniteCare initiatives begun last year to help seniors remain in their communities rather than have to move into nursing homes.
- Additional savings of $5.3 million are expected from implementation of the enhanced care coordination pilot project begun in state fiscal year 2007.
Elderly & Adult Services
- A GraniteCare Initiative to support seniors in their communities versus institutions. By the end of 2009, the number of seniors on Medicaid residing in nursing homes is projected to decline from the current annual average level of 4,701 to 4,420.
- Provides for a Cash and Counseling Program to allow seniors and disabled adults the ability to plan their own services and choose their own providers.
Medicaid Business & Policy
- Implements a Pay for Performance pilot program in the Manchester catchment area.
