You must complete shellfish application and pay appropriate fee(s). If you have a private water system,  a water test result within the last three months is required, and a completed Hazard Analysis Critical Control Point (HACCP) plan to be submitted with the application.  The Food Protection Program will conduct an inspection when required documentation is received and reviewed.
