Children and their families are eligible for services when a child has a diagnosis or suspected diagnosis of chronic illness or physical disability.
Adults with cancer, end state renal disease, cystic fibrosis, spinal cord injuries, hemophilia, and multiple sclerosis who require extensive medical treatment and are in need of financial assistance can receive services through the Catastrophic Illness Program (CIP). This program has its own application and eligibility guidelines.
A program-specific application must be completed to determine eligibility for services and, in general, the applicant must be:
- A New Hampshire Resident
- A US citizen, or the parent or guardian having primary responsibility for the applicant child is a US citizen
- Age 20 or younger for Special Medical Services
- Age 21 or over for the Catastrophic Illness Program

Eligibility for Children with Special Health Care Needs:
Assistance for health care and related services is provided to families who have:
- A child under the age of 21 years
- A child with an eligible medical condition and if family resources are less than $4000 and meet the Federal Poverty Guidelines for 185% of parents.
For families of children over 1 year of age, use guidelines for 300% of poverty.
Eligibility for the Catastrophic Illness Program:
Income eligibility is for:
1 Person - Gross Income $13,500
Family of 2 Persons - Gross Income $14,500
Family of 3 Persons - Gross Income $15,500
Family of 4 Persons - Gross Income $16,600
Note: Increase by an additional $1,000 for each additional person in the family.
Yearly award per client is $2,500 with a resource limit of $30,000 per family/household or individual.
