The Application for Special Medical Services (9 pages) is required to determine eligibility for medical and financial services to children with physical disabilities / chronic illnesses / special health care needs. The application may also be used to provide information and referral to community resources, chronic illness care coordination services, and nutrition, feeding and swallowing consultation.
