109.09 Reuse of Forms 800, 800HR, and 800Q After Denial or Closing (AAM)

Reuse * Forms 800, 800HR, or 800Q to open a denied application or to reopen a closed application if the denial or closing was:

• an error; or

• due to failure to provide verification, and the following applies:

- the applicant provides required verification within 30 days of the denial notice; or

- a former recipient provides required verification within 10 days of the end of the advance notice period.

The application date is the date the application was first filed. If the information on the application has changed, the individual must provide verification if required. The worker must document all changes. Conduct another face-to-face Application Entry interview if necessary.
