111.03 AR Signature Required (AAM)

The AR’s signature is required on BFA Form 778, Authorized Representative Declaration.

The AR must also acknowledge they have assisted the individual by signing all forms they have assisted the individual with completing.

If able, the individual must also sign or mark each form.
