Annual Child Care Immunization Report

Beginning each school year, all New Hampshire child care agencies are required to submit an annual Child Care Immunization Report to the NH Immunization Program by November 15th of that year. This is a requirement under NH State Statute 141-C:20-e, Immunization Reports which states, "Schools and child care agencies, whether public or private, shall make an annual report to the Commissioner relative to the status of immunization of all enrolled students." The NH Immunization Program (NHIP) will send out Annual Child Care Immunization Report instructions in September of each year.  To receive this information by e-mail, child care agencies must make sure that NHIP has a current e-mail address on file. 

Child Care Agencies have from September until November 15th of the same year to complete and submit their Annual Child Care Immunization Report.  Child Care Providers may begin completing this report upon receipt and submit it any time during the September to November 15th timeframe of that year.
