109.07 Reuse of Forms 800 and 800HR After Denial or Closing (FAM)

Reuse Forms 800 or 800HR* to open a denied application or to reopen a closed application if the denial or closing was:

• an error; or

• due to failure to provide verification, and the following applies:

- the applicant provides required verification within 30 days of the denial notice; or

- a former recipient provides required verification within 10 days of the last day of the advance notice period.

If the information on the Form 800 or 800HR* has changed, the individual must provide verification if required, and the worker must document the changes. Conduct another face-to-face Application Entry interview if necessary.
