109.05 Withdrawal of a Request for Assistance (FSM) |

An individual may voluntarily withdraw a request for assistance anytime before an eligibility determination is made.
• Document the reason for the withdrawal, if given.
• Advise the individual of the right to reapply at any time.
• Advise the individual of the right to reapply within 30 calendar days without filing a new DFA Form 800.
• If the withdrawal is not made in person or is made by an authorized representative, contact the applicant to verify the withdrawal.
