127.05 Food Stamp Identification Card SR 94-04, 04/94 (FSM-A)

Food stamp identification cards are used to identify the head of the household and authorized representative (AR) for receiving or using food stamps.

The head of household and the AR must both sign the card prior to using it. Obtain the signatures during the initial eligibility interview, if it appears the household will be eligible. Photograph the individual or AR for the card, if they consent.

Issue the card when the household is determined eligible. Never mail identification cards to the household in the food stamp envelope.

Issue replacement cards only when the following occurs:

. there is a change in the individual authorized to receive or use food stamps, or

. the original card is lost, mutilated, or destroyed.

Mark the card with the letter "M" if the individual is eligible and intends to use food stamps to purchase delivered meals. If the individual is only temporarily eligible for delivered meals, such as while convalescing, include an expiration date.

Mark the card with the letters "CD" if the individual is eligible and intends to use food stamps to purchase prepared meals at communal dining facilities.
