161.03 Required Verification for Redetermination/Recertification SR 09-11 Dated 11/09 (FSM-A)

During the interview, notify the individual in writing of any required information or verification needed to complete the redetermination/recertification, and the time frame for returning the information or verification. New HEIGHTS will automatically generate the AE0026, Verification Checklist, which explains what information or verification is needed and the date the information or verification is due. The New HEIGHTS-generated AE0026 is automatically generated anytime something is entered onto the Application Entry screen, marked "Not Yet Verified," and eligibility is run. If New HEIGHTS is unavailable, use the paper backup DFA Form 808, Proof Needed to Determine Your Assistance.

The individual must provide verification only of information that is incomplete, inaccurate, inconsistent, outdated, changed since the last eligibility determination, or otherwise questionable.

Do not re-verify deductions incurred by the recipient when:

• there have been no changes in the deductions; and

• the deductions have been previously verified by a third party.

Assist individuals in obtaining missing verification when:

• they are unable to provide missing verification due to circumstances beyond their control; or

• a request for information was sent or given to a third party and that party fails or refuses to cooperate.

If unsuccessful:

• document the case record indicating the households inability to obtain verification; and

• allow the individual to submit a written self-declaration statement to verify the missing information.

The individual must provide verification of all of the following:

• changes in income, including termination of employment, if the amount changed by more than $100* per month;

• changes in utility expenses if the source changed or the amount changed by more than $25 per month;

• new medical expenses, or changes in total recurring medical expenses of more than $25 per month;

• values of all vehicles, including those considered inaccessible resources;

• new social security numbers; and

• changes in interest-bearing liquid resource accounts.
