309.05 Completing an Application for an SSN SR 95-15, 07/95(FSM-A)

The following individuals must apply in person for an SSN at the Social Security Administration (SSA):

• aliens and naturalized U.S. citizens who cannot furnish an SSN, and

• U.S. citizens age 18 or over who have never been issued an original SSN.

Provide the above applicants with an SSA Form SS-5, Application for Social Security Card. The SSA will report the individuals SSN back to the Division of Family Assistance and generate a card.

If they dont already have an SSN, the following individuals must complete an Application for Social Security Card, but they are not required to apply in person at SSA.

• U.S. citizen under age 18.

• Any citizen who cannot produce a previously issued SSN.

Assist these individuals in completing the application and in obtaining verification of age, citizenship, and identity when they cannot do so alone. See forms instructions for the Social Security Administrations verification requirements.

Application for an SSN will generate an original social security card. However, when an original social security card has been lost, application for an SSN will generate a duplicate of the original.
