709.11 Replacing Coupons Lost in Certified Mail SR 94-04, 04/94 (FSM-A)

If a household reports that coupons delivered by certified mail were not received, take the following actions to replace coupons:

1.  The individual must write a statement on Form 422 that the coupons were not signed for by a household member or anyone residing with or visiting the household.

2.  The individual must submit a copy of the certified mail receipt from the post office. If the mailing was signed for by a household member, do not authorize a replacement.

3.  The individual must submit the certified mail receipt and the completed Form 422 within 10 days of reporting the nonreceipt. Otherwise, do not replace the coupons.

4.  Advise the individual of the time frames for replacing coupons. Replacement coupons lost in certified mail require 15 days to be issued, or 2 days from filing Form 422, whichever is later.
