709.13 Replacing Post Office Box Losses SR 94-04, 04/94 (FSM-A)

If a household reports that coupons delivered to a post office box were not received, take the following actions to replace coupons:

1.  The individual must provide the identity of the owner of the post office box and the identities and addresses of any persons having access to that post office box. These must be included on Form 422.

2.  All individuals-not just the head of household-with access to the post office box must sign the penalty warning on Form 422.

3.  Send Form 422 to BDM.
