709.15 Failure to Cooperate with the Post Office SR 94-04, 04/94 (FSM-A)

When the post office searches for lost coupons, it may need to request additional information from the individual. If the post office reports to the Bureau of Data Management (BDM) that the individual refuses to cooperate, take the following actions:

• Contact the individual. Require that they complete a Form 422 (or other statement of nonreceipt) even if they have already completed one. This is in case the individual received the coupons after completing the first affidavit of nonreceipt. The new Form 422 must answer the post offices questions.

• If the individual completes Form 422 and supplies the information requested by the post office, forward the Form 422 (or statement) to BDM with a copy of PS Form 1510, Rifling/Loss of Mail, on which the post office had originally complained of the individuals noncooperation. BDM forwards the package to the post office and monitors follow-up action.

• If the individual refuses to provide requested information, complete Form 104, Referral of Allegation, with a copy of Form 1510 to BDM. BDM forwards these to Office of Special Investigations (OSI). OSI notifies both BDM and the District Office of the case disposition.
