New Hampshire's Department of Health and Human Services (DHHS) is committed to the health of all Granite Staters, and is actively working on health care transformation. In support of this work, the State received a State Innovation Model (SIM) Design cooperative agreement from the Centers for Medicare and Medicaid (CMS) to develop a plan for improving health in New Hampshire. As part of this initiative, DHHS envisions developing a new regionally-based integrated health delivery model that will:
- Improve the quality and efficiency of health care delivery
- Expand the use of health information technologies
- Lead local population health improvement programs
Beginning in September 2015, DHHS is establishing a public process, informed by diverse stakeholder perspectives, to define a model for regional health care innovation across the state. The New Hampshire SIM will use a stakeholder-centered approach to engage policymakers, health care leaders, and consumers in the development of a plan through interviews, work groups, public meetings, evidence, and data.
