Provider Directory – Tips on Finding Your Doctor
The Provider Directory can help you choose a Health Plan. Use the Provider Directory:
- To find your doctor or the group they are a part of
- To see what Health Plan they work with
It is important to check the Health Plan to see how it best meets your needs – including care coordination and the incentives they offer to help you stay healthy.
You will get more results the fewer fields you fill in. Use these tips to get better search results.

You do not have to enter information in each field to search for your doctor.
TIP: Unsure about the spelling? Try the first few letters of the doctor's first OR last or the group name in the text box and select 'search.'
TIP: Make sure that you are not filtering your search by PCP if you want to find a specialist like an optometrist or pharmacy. Check to make sure that the "All" button is selected so that the listing does not exclude any providers.
Know which Health Plan you like best? Search for your provider by Health Plan, using the dropdown box.
TIP: Try to combine the information you know together – for example:
- Try the doctor's last name and the town
- Try the doctor's office phone number and the doctor’s first name
- Try the doctor's group name and write in the zip code
TIP: Using the information you know will get you closer to finding YOUR DOCTOR and which HEALTH PLAN they are in.

Still not able to find your doctor?
The Health Plans work to keep the most current provider listing published to the DHHS Provider Directory. That means that the DHHS or Health Plan Provider Directories change frequently. If you are still unable to find your doctor - search on each of the Health Plan websites or call their Member Services.
