305.01 Declaration of Citizenship or Alien Status SR 94-04, 04/94 (AAM-A)

One adult representative of a household applying for assistance must complete Form 752, Affidavit of Citizenship/Alien Status, to declare that each household member is a citizen or lawfully admitted alien. Adults must sign for individuals under 18 years old.

Exceptions:

• If there are no adult assistance group members, the applicant may sign for all members.

• An authorized representative may sign for an adult applicant with a severe physical or mental disability. If able to sign, however, the applicant’s signature or mark must appear on the form. An "X" mark must be witnessed.

Form 752 must be completed at initial application, when a new member joins the household, and whenever a change in citizenship or alien status occurs. Complete Form 752 before establishing eligibility for benefits.
