This Request for Applications (RFA) is published to solicit applications for the provision of services that increase the accessibility of comprehensive pediatric specialty care for children with special health care needs by:

• Increasing capacity for care coordination;
  
• Increasing the participation of families in planning and delivery of services;
  
• Increasing current services to children and their families;
  
• Enhancing the ability of established organizations to provide quality health care and family support; and
  
• Integrating new services to enhance the system of care and coordinate service delivery.
