This Request for Applications (RFA) is published to solicit applications for the provision of Complex Care Consultation, Clinic and Network services for children of ages from birth to 21 years who have complex health care needs. The selected Applicant will provide services to:

• Enhance comprehensive care for children with special health care needs (CSHCN);
  
• Increase the participation of families in planning and delivery of services;
  
• Increase current services to CSHCN and their families;
  
• Enhance the ability of established organizations to provide quality health care and family support; and
  
• Integrate services to enhance the system of care and coordinate service delivery.
