This Request for Applications (RFA) is published to solicit applications for Partners in Health (PIH) services that address the diverse needs of children and youth with chronic health conditions and their families, in order to assist clients with self-advocacy, accessing resources, navigating health care systems, and building skills to manage their children’s chronic illness through family-centered education and evidence-based family support.
