REAP is available to all older adults and their caregivers in their community! REAP is designed to help you regain quality in your life, to live a happy, healthy, and independent lifestyle. We offer you free confidential counseling and educational services to help you better understand and deal with the many life changes you may be facing. REAP counselors are available by phone or by appointment to provide you or your loved ones with information and resources on how to deal with life changes you’re experiencing including loss, grief, and other personal concerns.

Eligibility for REAP

  • NH residents aged 60+ and anyone living in Senior Housing,
  • Caregivers and family members of anyone eligible for REAP, and
  • Professionals who work with and / or on behalf of older adults in NH.

Exceptions: Permanent residents of long term care facilities and active clients of a NH Community Mental Health Center are not eligible for the REAP Program.
