Waiver Renewal Process
New Hampshire’s Department of Health and Human Services requests your input as the Bureau of Developmental Services (BDS) begins the In Home Support (IHS) Waiver renewal process Adobe Acrobat Reader Symbol. Every five years the Centers for Medicaid and Medicare Services (CMS) requires that the states renew their Home and Community Based Care 1915 (c) waivers.

The renewal process is lengthy and includes collaborative meetings with a wide range of stakeholders to gather input on the waiver. BDS is working with the Institute on Disability (IOD) to deliver three listening sessions statewide. BDS encourages people to participate in the listening sessions noticed below or to provide input by email or mail. Sharing what has worked and what needs attention helps inform waiver changes that will be available for public comment in Spring 2020.

In Home Support (IHS) Waiver Information
The In Home Support (IHS) Waiver Adobe Acrobat Reader Symbol provides Family Support/Service Coordination, Enhanced Personal Care Services, Respite, Environmental & Vehicle Modifications and Consultative Services to New Hampshire’s eligible children. The waiver is a participant directed and managed waiver meaning eligible individuals and or their guardians/families have the ability to move funds within an established budget to meet their unique needs, within service limitations as specified by waiver and or state regulation He-M 524.

Following is a brief description of the services within the current waiver:

Family Support/Service Coordination

  • Coordinating, facilitating & monitoring services provided under the waiver
  • Assessing & reassessing service needs
  • Assistance with recruiting, screening, hiring & training in home support providers
  • Identifying, providing information regarding & assisting families to access community
  • Resources and support
  • Development, review & modification of service agreements

Enhanced Personal Care

  • Personal hygiene, ambulation & movement
  • Nutritional care, elimination, assistance with/use of adaptive devices
  • Administration of medications
  • Activities that are directly supportive of skilled therapy
  • Communication, including use of assistive technology
  • Learning to make choices, accessing and using transportation,
  • Developing and maintaining personal relationships, participation in community experiences.

Respite

  • Consists of the provision of short –term assistance for temporary relief& support of the family.
  • May be AA arranged or Family Arranged (Service Limitation to 15% of budget)

Environmental &Vehicle Modifications

  • Installation of ramps and grab bars, Widening of doorways,
  • Modification of bathrooms and facilities
  • Outdoor Fencing (Service limitation, shall not exceed $2,500 /approx. 3,500 sq.ft)

Consultative Services

  • Evaluation training
  • Mentoring
  • Therapeutic recreation (service limit of $1,200 per year)
  • Assistive technology and /or special instruction (service limit of $ 1,500 per year and requires a state plan denial for waiver funds to be accessed)
  • Support & counseling regarding diagnosis and treatment of the individual to families.
