Eligibility – Housing Services

Residential Care Services

An individual qualifies for Residential Care Services through the Home and Community Based Care for the Elderly and Chronically Ill (HCBC-ECI) Medicaid waiver program. This is a four-step process.

  • An individual must apply and be determined eligible for Medicaid. This process takes place at DHHS district offices and includes a financial eligibility determination.
  • An individual must apply for the HCBC-ECI program. This process starts at a NH ServiceLink Resource Center. An individual must have medical needs that require nursing facility level of care. A Bureau of Elderly and Adult Services (BEAS) health care professional determines if an individual meets the medical criteria for nursing facility level of care and whether HCBC-ECI is an appropriate and cost effective option.
  • When an individual is found to meet the nursing facility level of care criteria, he or she meets with a nurse and a case manager to determine eligibility for HCBC-ECI services.
  • The nurse, case manager and individual jointly develop a service plan. It is critical that services be locally available to support the service plan. Case managers meet with the individual and consult with BEAS nurses on an on-going basis.

Supported Housing Services
An individual who is eligible for the Home and Community Based Care for the Elderly and Chronically Ill (HCBC-ECI) Medicaid waiver program may request Supported Housing Services. Additional criteria that apply to this service are:

  • Must meet the requirements for Residential Care Services.
  • Live in a municipal or public housing facility that participates in the Supported Housing Support Services program
  • Be unable to perform at least three activities of daily living, such as bathing, dressing, eating or toilet use.
