New Hampshire HospitalScheduling of New Hampshire Hospital conference rooms and Howard Recreation Center Auditorium and Gymnasium shall be arranged according to established guidelines. Scheduling of the softball and soccer fields on the Hugh Gallen, State Office Park South Campus is done through the Department of Administrative Services, Bureau of Facilities Assets Management at (603) 271-9501.

A Request for Facility Use and an Indemnity Agreement must be completed. The forms are below. When complete, please fax them to (603) 271-5395.

All requests for repeating activities must be received three weeks in advance. All requests for facility use shall be reviewed by the Director of Rehabilitation Services prior to use. Once a decision has been made, the requestor will be notified by e-mail and, if approved, a copy of the procedures and regulations governing use of the area will be attached.

No single organization or agency shall monopolize the use of the available resources. State Agency sponsored business functions shall be given priority as determined by the Director of Rehabilitation Services. Activities that may cause considerable wear and tear on the facility will not be allowed.

If it becomes necessary for the Hospital to rescind use of space, the organization using the area shall be given as much advance notice as feasible of the change in the schedule. Every effort shall be made to reschedule the organization for any time missed.

All agreements to be signed shall include a starting and termination date and an agreement to abide by the Hospital policies pertaining to drugs, alcohol, smoking weapons and parking.
