NH Immunization Requirements for Child Care

New Hampshire law He-P 301.13-15 requires that all children enrolled in any school, pre-school, or child care have certain immunizations to protect them and those around them from vaccine preventable diseases. In addition, schools and child care providers must collect and review the immunization records of enrolled children and submit an annual immunization report to the DHHS (RSA 141-C:20-e).

Child Care2018 Annual Child Care Immunization Report

The 2018 Annual Child Care Immunization Report will be released in early to mid-October and will be due November 15, 2018. If NHIP has an email address for your licensed child care, you will receive the report electronically. Otherwise, it will be mailed to you. Please contact the NHIP if you wish to update the contact information for your child care. This report is required by NH State Statute 141-C-20-e.
