The NH Immunization Program (NHIP) developed the New Hampshire Child Care Provider's Guide to Immunizations Adobe Acrobat Reader Symbol, to assist you in understanding immunizations and provide you with helpful tools to track and report required vaccines.

NH Immunization Requirements for Child Care

New Hampshire law He-P 301.13-15 requires that all children enrolled in any school, pre-school, or child care have certain immunizations to protect them and those around them from vaccine preventable diseases. In addition, schools and child care providers must collect and review the immunization records of enrolled children and submit an annual immunization report to the DHHS (RSA 141-C:20-e).

2018 Annual Child Care Immunization Report

Child CareEach year, every public and non-public child care in New Hampshire is required to complete and submit the Annual Child Care Immunization Report by November 15th.

The electronic child care report link is emailed to child care facilities in early October. If you did not receive an email, please complete the online report now.

You may download a copy of the 2018 report form Adobe Acrobat Reader Symbol to use as a worksheet. Although submitting the report online is preferred, you do have the option of completing the childcare report on paper and faxing it to 603-271-3850, or mailing to the address noted on the forms.

It is important to only complete the online report once! If you have received the link to the online report and have started or submitted that report, do not fill out another online report. Also, if you have completed the online report, you do not need to send in a paper copy.

This report is required by NH State Statute 141-C-20-e.
