903.05 Required Verification for Wait List Exemption (FAM)

Verify exemptions from the wait list by doing the following:

If notified by DCYF that a child is exiting preventive or protective services, write a case comment in New HEIGHTS indicating that the FSS has received confirmation that this service has closed in the past 92 calendar days. Include the date  the preventive or protective services closed in the comment. Contact DCYF Provider Relations for confirmation, if necessary.

If TANF cash has closed within the past 92 calendar days, add a case comment with date of TANF cash closure.

If the single parent is placed on orders or deployed for military service, require verification from the branch of the military service that the individual will be outside of New Hampshire for more than 30 calendar days.

If a family is currently experiencing homelessness or has experienced homelessness within the past 92 calendar days, require a self-declaration stating that:

the family is currently homeless or was homeless within the past 92 calendar days; and

the parent(s) is currently participating in an employment, training,education, recipient of NHEP or FAP and participating in a mental health or substance misuse treatment program  or job search activity.
