903.07 Required Verification to Grant a Good Cause Extension (FAM)

If a family meets  good cause criteria for failing to be linked to a child care provider within the required 30 calendar days from the date of application,  30 calendar days from the child care provider link end date, or 30 calendar days from when the child was released from the wait list, verify by the following means:

Verification from the Child Care Resource and Referral Agency is required when aa family is unable to find child care with the assistance of  Child Care Resource & Referral  during the 30 day period. Verification must state  the parent has been searching for child care with the assistance of Child Care Resource & Referral and child care for the specific child has not been identified. There will be no extensions beyond the additional 30 calendar days.

If a parent or child has had an in-patient hospital stay during the 30 day period, require the hospital record, the discharge statement or a statement from the attending physician.
